 2. Identification of the removed or disabled material and its location before removal.
 3. A statement, under penalty of perjury, that you have a good-faith belief the content was removed as a result of mistake or misidentification.
 4. Your name, address, telephone number, and a statement consenting to the jurisdiction of the Federal District Court in your area, and that you will accept service of process from the complainant or their agent.

Repeat Infringers BVM+ reserves the right to terminate, without notice, the accounts of users determined to be repeat infringers or who violate this policy. 4. Removal of Infringing Content Upon receiving a valid DMCA notice, BVM+ will remove or disable access to the allegedly infringing content and notify the user who uploaded it. 5. Misrepresentation Knowingly submitting false or misleading information in a DMCA notice or counter-notice may result in legal liability under 17 U.S.C. §512(f).
